New Patient Forms

We ask all new patients to complete our new patient forms prior to the first visit. Your answers on the new patient form provide Dr. Andrea with details about your current symptoms, the general state of your health, your family medical history, health history and daily activities.

Please complete the new patient forms before your first visit. Doing so will save time and allow us to see you on time.

You can download and print the forms below and bring them to your first appointment. If you don’t have access to a printer, we can send you the form by mail or you may arrive 30 minutes early for your first appointment to complete the forms.

What To Expect in Your First Appointment

 During your first visit, Dr. Andrea will ask you questions about your history and health goals, and discuss the information you’ve shared in the new patient form. We encourage you to openly share your story and articulate during this conversation what you believe is preventing you from living your best life. Your answers will help Dr. Andrea create an effective chiropractic care plan that can address the symptoms you’re experiencing.

After your conversation with Dr. Andrea about your goals and health history, you’ll be given the NeuroInfiniti Stress Response Evaluation, which is a non-invasive, computerized evaluation stress response scan. The stress response evaluation helps us assess your brain and nervous system by measuring how your brain and body respond to stress. If needed, Dr. Andrea may also check neurological reflexes, blood pressure, range of motion, muscle tension and perform other functional neurological assessments.

Please plan to spend approximately an hour in our office during your first visit. Our office strives to keep waiting times to a minimum, so we ask that all patients arrive on time or a few minutes early for their appointments.

Report of Findings

 You will be scheduled to meet with Dr. Andrea again within a few days of your initial visit to discuss the results of the stress response evaluation and your initial visit. You may invite anyone in your family to accompany you, if you choose. During this report of findings visit, Dr. Andrea will provide a detailed analysis of your stress response evaluation and discuss options for a personalized care plan that can address any symptoms you may be experiencing.

Good health is based on your brain balance, nutrition, regular exercise, high quality supplementation and a variety of stress relief techniques. Dr. Andrea will offer guidance on how to strengthen each of these pathways to greater health during the report of findings visit.
